The Home and Community Based (HCS) Program is an individualized Medicaid Waiver program that is an alternative to institutional and traditional based care programs. This program caters to the needs of people with Developmental Disabilities in their living environments.

At Axcess Healthcare LLC, you can be assured of the quality of our services. We are licensed and contracted by Texas Health and Human Services to give qualified individuals with Developmental Disabilities in relation to the HCS program. You can also depend on our staff to provide your loved ones the care that they need since they have 15 years of experience in caring for Developmentally Disabled clients.

Eligibility Requirements:

  • Must have the diagnosis of Intellectual Disabilities or Related Conditions.
  • Must have a current level of care and level of need.
  • Must have a current individual plan of care.
  • Must be currently receiving SSI with Medicaid.

The Home and Community Based (HCS) Program includes the following services:

  • Residential Services

    This service enables people living in group homes to have access to quality care and at the same time be given the opportunity to learn healthier coping mechanisms and skills that they may find useful for the rest of their lives. Group homes are also known to be effective for people recovering from alcohol addiction.

    The staff in care home are there to help 24/7 and assist the consumers with daily living skills, transportation to appointments, meal preparations, and community outings.

  • Supervised Living

    Unlike Residential Services, group homes that provide Supervised living have residents that do not require round-the-clock assistance. However, caregivers are available whenever their services are needed.

  • Supported Home Living

    This service is provided to individuals who are living in their own homes or with their family. The staff visits the client and supports them whenever they are needed. The level of assistance that they receive is based on their needs.

  • Foster/Companion Care

    Individuals who are receiving this type of assistance is living with their families or being cared for foster families.

  • Case Management Services

    Case managers are assigned to individuals to monitor the implementation of Individual Service Plan, Individual Plan of Care and record the delivery of all services as required by HHS.
