HCBS-AMH provides people Mental health or substance abuse the chance to receive assistance that is suitable for their overall needs. Its various services can ensure that everything that the clients need is available.
Home and Community-Based Services-Adult Mental Health is a program that provides home and community-based services to adults with serious mental illness. The program provides an array of services, to match each person’s needs. This helps the person to live and experience successful tenure in their chosen community. Services are designed to support long-term recovery from mental illness.
Individuals determined eligible shall be enrolled in the program by DSHS.
Program Eligibility Criteria:
Initial Eligibility Criteria:
- Adult (18 years of age and older) with a diagnosis of serious mental illness (SMI);
- Meet financial eligibility (150% FPL or below); and
- Meet state-defined needs-based criteria
Additionally, the individual must meet one of the following:
- During the five years prior to their referral:
- reside three or more (cumulative or consecutive) years in an in-patient psychiatric hospital; and
- be Medicaid eligible
- During three years prior to their referral:
- two or more psychiatric crises (i.e., inpatient psychiatric hospitalizations and/or outpatient psychiatric crisis that meet in-patient psychiatric criteria);
- four or more arrests; and
- have active Medicaid
- During the three years prior to their referral
- two or more psychiatric crisis (i.e., in-patient psychiatric hospitalizations and/or outpatient psychiatric crisis that meet in-patient psychiatric criteria);
- fifteen or more total ED visits; and
- have active Medicaid
