Home and Community Based Services-Adult Mental Health is a program that provides mental health and substance use recovery services for eligible adults with serious mental illness. The services are provided at the person’s home or where they live or in a group home setting. HCBS-AMH has recovery managers who are trained in the Person Centered Recovery Planningmodel. The recovery manager helps people in their recovery from mental illness in liaison with provider agency to provide the program services. Program Eligibility Criteria includes
Initial Eligibility Criteria:
- Adult (18 years of age and older) with a diagnosis of serious mental illness (SMI);
- Meet financial eligibility (150% FPL or below); and
- Meet state defined needs-based criteria
Additionally, the individual must meet one of the following:
- During the fiveyears prior to their referral:
- reside three or more (cumulative or consecutive) years in an inpatient psychiatric hospital; and
- be Medicaideligible
During the threeyears prior to their referral have:
- two or more psychiatric crises (i.e., inpatient psychiatric hospitalizations and/or outpatient psychiatric crisis that meet inpatient psychiatric criteria);
- four or more arrests; and
- have active Medicaid
